The report provides information submitted by applicants for the awards, including program descriptions and contact information for the finalists and the state winners. It also includes a summary of the trends and issues regarding libraries serving the community health needs through things such as outreach, partnerships, health literacy, and web initiatives. (Reported in special issues of Library Trends 2004 and 2005 and a supplement in the Journal of the Medical Library Association 2005).
